PATH's Horizon Program

Our 7th-grade students have been enrolled in a program through Positive Approach to Teen Health titled Horizon. The Horizon program is a voluntary opportunity for students to learn about risky behaviors and ways to avoid them. Your consent is requested for your 7th-grade student to voluntarily participate in the Horizon program survey. Please only submit the form that was sent home with your student if you do not want your child to participate in the survey.

PATH is a non-profit organization that provides the Horizon program, using our educators to provide instruction.  All PATH programming is free to our local schools and has a signed general MOU from the superintendent.

PATH Horizon  is committed to continuing to be in compliance with state legislative guidelines. The Horizon curriculum will be made available upon request at the main office of the school (building principal) and at the office A Positive Approach to Teen Health (PATH) located at 3391 Airport Rd. Portage, IN 46368
